Insurance Accepted
We participate in most major insurance company health plans. Please contact our office to verify our practice’s participation in your specific health plan if you have questions.
We will gladly file claims with your insurance company if we are contracted with your carrier. Please be prepared to pay all copays, deductibles, and other services not covered by your insurance company at the time of service.
Please become familiar with your health plan and what services are covered, as every policy is different.
Additional Charges
We may charge additional fees for the following services.
